To take advantage of the offer, head to the Starbucks-Masterpass promo page.  If you don’t already have a Masterpass account, you can sign-up for one there.

Then, add a $10 gift card to your cart and you’ll see the extra $5 promotional gift card on the checkout page.

The $5 Promo Card Will Show Up Automatically When You Go to Checkout

Some folks have reported only seeing the option to checkout with a Capital One Mastercard via the promotion page.  If you run into this issue, open the promo page in incognito mode and you should be able to use any Mastercard you want.
